By bringing together enterprises in care delivery, healthcare incumbents, and vendors, we’ll share successes in recognizing and supporting the work caregivers do to support patient health & outcomes.
SPEAKERS
Dr. Namita Seth Mohta is a distinguished physician executive with expertise in health care delivery transformation and innovation. She received her medical degree from Yale School of Medicine and completed her residency at Brigham and Women’s Hospital, where she has practiced as an academic hospitalist since 2006. She held the role of Director of the Serious Illness Care Program at Ariadne Labs, improving care for patients with serious illnesses. She serves as the Executive Editor for NEJM Catalyst Innovations in Care Delivery, Associate Faculty at Ariadne Labs, and Assistant Professor at Harvard Medical School. Her leadership roles extend to founding teams for Population Health and Accountable Care Organizations, and she has collaborated with multiple startups, demonstrating her ability to integrate traditional healthcare delivery with technology.
Jessica C. Guthrie, M.Ed is a passionate advocate and thought leader in caregiving, with over a decade of experience in the field. As a primary caregiver for her mother with Alzheimer’s Disease, she has a deep understanding of the challenges faced by families and emphasizes the need for collaboration among caregivers, families, and healthcare professionals. Through her platform, Career & Caregiving Collide™, Jessica provides insights on balancing careers with caregiving responsibilities and amplifies diverse caregiving narratives. She has recently launched Jessica C. Guthrie Caregiving Consultancy, a pioneering consultative service dedicated to helping companies, brands, and healthcare organizations develop more comprehensive support strategies that amplify and validate the critical experiences of family caregivers.

As the Chief Medical Officer of Government Products, UPMC Health Plan, Jatin provides strategic, clinical, and policy leadership and oversight of healthcare services for Government Products at UPMC Health Plan. Jatin is an Associate Professor at U Mass Chan Medical School in the Department of Population and Quantitative Health Science and is board certified in Internal Medicine, Geriatric Medicine, Hospice and Palliative Care and Clinical Informatics.
Chris Johnson is the founder and CEO of Bluebird Kids Health, a pediatric primary care organization serving over 20,000 patients. Through value-based care partnerships with payers, Bluebird Kids Health delivers comprehensive care by integrating physical, mental, and social health services. Its innovative clinical model is supported by a population health technology platform, which ensures proactive access to care tailored to the needs of each child. Before founding Bluebird Kids Health, Chris served as the Chief Executive Officer of Landmark Health, an organization specializing in in-home primary care for seniors with serious illnesses in value-based partnerships with Medicare Advantage plans. Under his leadership, Landmark expanded its operations from 10 to 38 states and grew its patient base from approximately 150,000 to 700,000. Chris also guided the organization through its integration with Optum, where Landmark became a cornerstone of Optum’s Home and Community portfolio. Chris resides in Newton, Massachusetts with his wife, Devon, and their three young children. In his free time, he enjoys exploring the outdoors with his family.
Dr. Robert Moskowitz is the Chief Medical Officer for Contessa Health, a company focused on high acuity care in the home. Contessa has partnerships with both health care systems across the U.S. and directly with payers to provide a continuum of in-home opportunities for patients and their families, most notably: Hospital-at-Home, Skilled Nursing-at-Home, and Palliative Care-at-Home. Contessa Health, established in 2017, creates longitudinal experiences under value-based and risk-based arrangements in addition to those under CMS waiver guidelines. In his role as CMO, Dr. Moskowitz oversees all clinical and quality aspects of their suite of services.
Gavin Baumgardner, DO, MHA serves as Contessa’s Vice President and National Medical Director of Palliative and Complex Care. With extensive experience in geriatric medicine, palliative care and hospital medicine and more than 20 years in the healthcare field, Dr. Baumgardner has worked with patients across the full continuum of their care. He is passionate about palliative care and supporting new models of care that effectively partner with healthcare teams to deliver palliative care in patients’ homes and improve quality of life for patients and caregivers.

Sarah Lejfer oversees the innovation program at Boston Children's Hospital, where she has been for 7+ years. Her team drives digital health incubation and commercialization efforts, strategic partnerships, and novel builds & deployments leveraging emerging technologies within the hospital. Before joining Boston Children’s, Sarah spent some time in product at a digital health startup and had a relatively short stint in healthcare consulting. She received both her Master’s in Public Health and her undergraduate degree from the University of Michigan in Ann Arbor (go blue!).

Mihir M. Kamdar, MD, is Head of Clinical Delivery for Tuesday Health, a value-based care supportive & palliative care company. He is the former Section Head of the Division of Palliative Care at Massachusetts General Hospital, Assistant Medical Director of Telehealth at Mass General, and the Director of the Mass General Cancer Pain Clinic. Trained in Internal Medicine, he is dual-fellowship trained in both Palliative Care and Interventional Pain Medicine. His clinical and research interests center on the design of innovative, early integrated models of supportive & palliative care. He serves on the teaching faculty at Harvard Medical School
Ian is a Partner at Flare Capital, where he led investments in Knownwell and played key roles in Inbound Health, RightMove (Board member), Cayaba Care, and Oshi Health. He also spearheaded Flare Scholar Ventures investments in Adni, EarlyBird Education, Narrable, and Vincere Health. Before joining Flare, Ian was SVP and a founding member of CareAllies, Cigna’s multi-payer provider services arm, where he developed technology-enabled services and strategies for population health and home-based care. Prior to CareAllies, he held leadership roles at Cigna's US Strategic Operations team. Ian co-founded startups like XcelDx, which partnered with Scanwell Health (acquired by BD in 2021), focusing on smartphone diagnostics. He began his career at McKinsey & Company, advising healthcare clients globally, and continues to mentor innovative healthcare startups.
Joe leads growth and operations at Synapticure, a provider of comprehensive specialty care to individuals and caregivers living with neurodegenerative diseases. He is responsible for scaling the delivery of Synapticure's telehealth and in-home services in all 50 states including via the CMMI Dementia GUIDE program, and structuring partnerships with payers and providers to engage families and improve costs and outcomes. Joe’s career has focused on building and operating innovative care delivery models for seniors, a passion fueled and informed by his experience as a caregiver for close family member living with dementia. He previously led new markets at Cityblock Health, and strategy and partnership development at Evolent Health. Joe received his BA from the University of Pittsburgh and MS from Carnegie Mellon, and lives in Alexandria, VA with his wife and daughter.
Barry J. Jacobs, Psy.D., is a clinical psychologist, family therapist, and a Principal in the Philadelphia office of Health Management Associates, a national healthcare research and consulting firm. A former journalist, he is the author of The Emotional Survival Guide for Caregivers (Guilford, 2006), the coauthor of AARP Meditations for Caregivers (Hachette, 2016), AARP Love and Meaning After 50 (Hachette, 2020), and the forthcoming The AARP Caregiver Answer Book (Guilford, 2025) and has written a self-help column for family caregivers on AARP.org since 2014. In the past 20 years, he has given over 700 local, national, and international presentations on family caregiving to family caregivers, physical and behavioral health providers, and care managers. Barry was the primary caregiver for his mother and step-father, both of whom had dementia.
Jessica Kim is co-founder & Executive Chairwoman of ianacare, on a mission to encourage, empower, & equip family caregivers after her experience caring for her mom with pancreatic cancer. She is a 3x startup founder, honored as Crain’s Top 40 under 40, Forbes Next 1000, and Woman in Digital Health Award 2020, served as the inaugural EIR & Presidential Council advisor at Brown University, & Venture Partner at Praxis Labs. She graduated from Brown University & holds an MBA from Northwestern University’s Kellogg School of Management
Steven Lee has scaled companies from the garage stage through IPO, receiving awards including Emerging Tech NEVY, WSJ Top 50 Start-ups, and MITX Technology Award. He began his career working on the first commercially viable speech recognition systems at Nuance and SpeechWorks (now Microsoft). He holds a Master's and Bachelor’s degree from MIT. After confronting his grandfather’s Parkinson’s diagnosis, he became passionate about using technology to address gaps in caregiver support.
Sumit is a serial entrepreneur focused on digital health innovation, having co-founded and grown five companies over the past three decades. He is currently the Chairman, CEO, and founder of Cherish, which develops advanced radar-based sensor platforms and AI to revolutionize health and safety monitoring. Sumit also serves on the HIMSS board and is Chair of the HIMSS Nominating Committee, known for his expertise and visionary approach in the field.
Vanessa has developed a equity and quality of care framework to help healthcare and community organizations assess complex operational problems and devise strategic, quality improvement, and training initiatives to optimize outcomes and satisfaction among employees, partners, and patients. She's the co-founder at JVS Health and CEO & President of ventures focused on women's health and value-based care. Vanessa leverages her years of experience as a biomedical engineer and quality improvement executive in a multi-billion dollar health system to make abstract challenges concrete, identifying inputs impacting progress and actionable plans to move forward. Vanessa has anchored her mission on her personal story to improve healthcare access to underserved communities, especially in the space of dementia. Over the years, Vanessa has helped her employers and clients earn millions of dollars in revenue through initiatives ranging from creating a training-coaching program for 3,000 healthcare providers to meet Medicare quality metrics to interventions focused on setting standards of care to programs focused on engaging at-risk populations in the community. Vanessa graduated from Columbia University’s Fu Foundation School of Engineering and Applied Science with a BS and MS in Biomedical Engineering, specializing in Diagnostic Imaging. She is also certified in Quality Engineering and Quality Management and Organizational Excellence by the American Society for Quality. For her outstanding contributions, she has been awarded the 2018 Becker’s Hospital Review “Rising Stars Under 40” and 2017 Modern Healthcare’s “Up and Comers” Awards. Vanessa is also a certified life coach and passionate mentor with the Bowery Mission’s Mont Lawn City Camp Leadership Academy.
Dr. Carty is a seasoned healthcare executive. She is currently President and Chief Medical Officer at myLaurel, the leader in providing high-acuity, high-complexity care in patients’ homes. Previously, she served as Vice President, Network Performance at Blue Cross Blue Shield of Massachusetts, where she led the company’s initiatives with Massachusetts-based physician groups to measure and improve healthcare quality, safety and efficient outcomes. In addition, she served as President and Chief Medical Officer at Ready, partnering with at-risk providers, health plans and hospital systems to provide an innovative in-home care model. Her leadership extends to the role of Vice President of Medical Affairs at Good Samaritan Hospital, the largest hospital in the Steward Healthcare network and an original Pioneer ACO and Director of Clinical Operations Improvement at Brigham and Women’s Hospital in Boston. She also held a leadership position as Associate Chief Health Officer at Cityblock Health.
Jen's journey with Coral Care began with a personal mission. Frustrated by the challenges of finding quality pediatric care for her daughter, she recognized a systemic problem that needed to be solved. Coral Care was born out of this passion. Jen's vision was to create a platform that connected parents with top-tier developmental therapists, offering a simple and accessible solution to a complex problem. Inspired by her own experience, she aimed to break down the barriers that often prevent families from accessing essential care. Before founding Coral Care, Jen held product leadership roles at startups and global companies like Bose, Samsung, and Weight Watchers. At Coral Care, Jen is committed to empowering families and supporting pediatric specialists. Through innovative technology and a commitment to quality care, Coral Care has already provided care to families across the Northeast, driven real outcomes, and transformed how pediatric therapy is delivered. Jen has a BS in Mechanical Engineering from the University of Rochester, an MS in Engineering from Brown University, and an MBA from Babson College. She currently resides in Westchester, NY, with her husband, two daughters, and two dogs.
